The aim of screening is to detect the asymptomic cancer at its early stages and define the possible treatment for it. In order to evaluate effectiveness of the screening it is required to conduct the control examinations, preferably MRI with the function Atlas—screening of the whole body. Screening should lead to the decrease of mortality from detected cancer forms. Intermediate features of method effectiveness are decrease of widespread cancer form detection frequency, increase of early stages cancer form detection frequency, and improvement of survival. However, it is impossible to evaluate the screnning only on the basis of the survival. The cancer form, planned for screening, should represent the important problem of healthcare in Ukraine because of the high level of morbidity and mortality after it.
With appearance of “dimensional” tomographs it has become possible to examine the whole body (PET-analogue). The main benefits if tomography of the whole are: fast speed; high informational load; and accuracy.
The examination usually lasts for 1 hour. Tomography of the whole body involves 3-D visualization of the brain, head vessels, neck, lungs, heart, organs of abdominal cavity and of small pelvis, prostate, spine and mammary glands.The program whole-body screening—oncoscreening allows to detect the asymptomatic forms of oncological diseases of different localizations at early stages—including multiple metastases of 1 mm.
The tomography results are given the next day in the form of printed films and the films on CD/DVD – disk. The clinician of MRI and CT examinations prepares the conclusion and consults on the results of screening, gives recommendations about the further observation. 1. What is the whole-body screening?
In general, whole-body screening creates an image of whole body in 3-D. Screening means diagnostics of the whole body regarding the presence of oncological diseases in a case when there are no vivid symptoms.
2. What is a combination of MRI / CT scannings during screening?
A combination of MRI/CT scannings during the program of detection of early asymptomatic oncological forms of diseases of different localization during screening layers two images, received during the MRI and CT examinations in limits of one procedure. Program (PET-analogue) on the MRT creates color photographs of chemical change (metabolic activity) in tissues. Hence cancer tumor, as a rule, is more active than the normal tissue, the first appears in different colors on the PET-analogue MRI-program.
Combination of CT and MRI in screening can give fuller picture of the tumor localization, its growth, and also it allows to conduct the analysis of the tumor treatment effectiveness, than CT or MRI can do on their own. The screening test should be maximally sensitive and specific. In the literature there are published the results of 8 randomized controlled examinations regarding the investigation of mammographic screening effectiveness. These investigations showed that mammographic screening leads to decrease of mortality from the cancer of mammary glands among the women aged 50-70 years. At the same time screening of women aged less than 50 years, probably, would be not effective. For screening of lung cancer previously it has been suggested to use X-ray examination of chest and cytological examination of sputum. These methods were approbated in 4 randomized controlled examinations and proved ineffective. It has been shown that theirs usage does not decrease mortality from the lung cancer. At this moment there goes on the approbation of spiral computer tomography methods. Big hopes are also put onto the detection of early molecular lung cancer markers.
